Atlas Hémicolectomie droite 1

Periumbilical incision.

A. The duodenum behind the right colon.

B. Superimposition of the right colon, duodenum and right kidney.

C. The right kidney and ureter behind the right colon.

Atlas Hémicolectomie droite 3

Example of vasculature of the right and transverse colon : the middle colic, right colic and ileocolic arteries.

Same presentation with the right and transverse colon.

Atlas Hémicolectomie droite 4
Atlas Hémicolectomie droite 5

Right and transverse colic lymph nodes. In a centripetal direction : the epicolic lymph nodes, paracolic lymph, lymph nodes along the ileocolic, right colic, middle colic arteries and superior mesenteric artery.

The omentum is elevated from the right part of the transverse colon.

The right colon is mobilized from its retroperitoneal attachment.

The right division of the middle colic artery, the right colic and ileocolic arteries are divided at their origin so as to remove all the lymph nodes of the right colon.

The distal ileum and transverse colon are stapled.

The ileocolic anastomosis is performed. It can be hand-sutured (as represented) or stapled according the institutional policy.
